Using Microsoft Forms for Surveys and Quizzes

Create surveys, quizzes, and polls using Microsoft Forms and analyse results in Microsoft 365.

Microsoft Forms — Surveys and Quizzes

Microsoft Forms is available to all Microsoft 365 users at **forms.microsoft.com**.

### Creating a Survey

1. Go to forms.microsoft.com → **New Form**

2. Enter title and description

3. Click **Add new** to add questions:

- Choice (single or multiple)

- Text (short or long answer)

- Rating (star or number scale)

- Date

- Ranking

- Likert (agreement scale)

- Net Promoter Score (NPS)

4. Reorder questions by dragging

### Creating a Quiz

New Quiz → Add questions → For each question, set the **correct answer** and **point value**

Forms automatically scores submissions.

### Branching (Conditional Questions)

Show different questions based on a previous answer:

Question → three dots → Add branching → Set rules

### Sharing Your Form

Click **Share** → copy link, or share via:

  • Email
  • QR code
  • Embed code (for websites)
  • Collaborate: share for co-authoring with colleagues

### Collecting Responses

Responses tab → See summary charts and individual responses

Export to Excel: Responses → Open in Excel → full spreadsheet of all answers

### Response Settings

  • Limit to one response per person (requires sign-in)
  • Set start and end date
  • Accept responses from: anyone vs only people in your organisation

### Sending Results

Share results summary link with respondents so they can see aggregated data.
